Reporting EHS Incidents, Injuries, and Illnesses

All faculty, staff, and students have a role to play - ensuring people under their direction have all that they need to work and study safely, using good judgment and adhering to environmental health and safety precautions, and looking out for each other to avoid injuries, illnesses and environmental harm.  When something goes wrong, almost goes wrong, or when something just does not seem safe, it is important to report your concerns to your supervisor and/or to EHS.

An EHS incident is an event that results in an injury, illness, or exposure to an individual, damage to equipment, or is considered a near miss. A near miss is a potential hazard or an unplanned event that did not result in an injury, illness, exposure or extensive damage, but had the potential to do so. Examples of EHS incidents include injuries, chemical spills, fires and smoke events.

EHS Incident Reporting Procedures

An EHS incident is an event that results in an injury, illness, or exposure to an individual, damage to equipment, or is considered a near miss. A near miss is a potential hazard or an unplanned event that did not result in an injury, illness, exposure or extensive damage, but had the potential to do so.

  • Contact Public Safety at (518) 276-6611 or 911 if you are in need of emergency assistance
    • Public Safety is available 24 hours a day
  • All EHS Incidents that occur involving employees, student employees, visitors, and contractors on institute properties must be reported to EHS by completing an EHS Incident Report
  • If an EHS incident involves an injury, illness or exposure, employees (including student employees) must report the incident to their manager/supervisor immediately. The incident must be reported to Human Resources within 24 hours.

  • Contact Public Safety at (518) 276-6611 or 911 if you are in need of emergency assistance.
    • Public Safety is available 24 hours a day
  • Complete an EHS Incident Report
  • Following the submission of the EHS Incident Report form, please report the incident to Human Resources within 24 hours, which is crucial for meeting notification requirements and time sensitive follow-up actions
